This audit concluded that city’s Equal Employment Opportunity and Diversity (EEO) Office needs to improve its documentation and recordkeeping of the investigation process. The audit also determined that the EEO Office’s database used to track EEO complaints is inaccurate and incomplete. Because the data was not reliable, the auditors could not evaluate the timeliness of the investigation process. The audit includes recommendations to improve the EEO Office’s investigation documentation, records, and communications and the accuracy, completeness, and use of EEO data. Implementation of the recommendations should improve the overall efficiency and management of the investigation process.
