Birth and death certificates
You can request a Missouri birth or death certificate from the City of Kansas City, Missouri Vital Records Office by mail or online. If you or your child received his or her immunizations at our facility, you may also request copies of those records records through our office.
Birth and death certificates may be accessed at our primary location at 2400 Troost Ave., or at our Northland location, 4420 N.E. Chouteau Trafficway. Our primary location accepts cash and debit or credit cards, and our Northland location accepts checks and debit or credit cards. We accept checks and money orders for mail requests. Please make checks payable to Vital Records.
Who may request and receive copies of vital records?
- The primary person named on the vital records.
- A member of the immediate family, including includes wife, husband, son, daughter, mother, father, brother, sister, grandmother, grandfather, grandchild, aunt, uncle, niece and nephew. This does not include cousins.
- Others: guardian who shows guardian paperwork, foster parent who furnish custody papers, step-parent and attorney acting on behalf of the registrant or family.
