City of Kansas City Title VI Complaint Procedure

Title VI of the Civil Rights Act provides that no person shall, on the basis of race, color, or national origin, be excluded from participation in, be denied the benefits of, or be subjected to discrimination under any City program or activity that receives federal funding. If you have a Complaint under Title VI, complete this form and submit it to the Civil Rights & Equal Opportunity Department (CREO), Title VI Program, 414 E. 12th St, 4th Fl., Kansas City, MO, 64106. 

You may also file your complaint online by downloading and visiting the City's myKCMO app on your smart phone, or by clicking on the KCMO 311 icon to the right side of the screen. You may also call
3-1-1 and tell the Customer Service Specialist that you want to file a Title VI complaint. They will walk you through the process. Your form will be sent to CREO, where an intake officer will contact you for additional information.
